LOST/DESTROYED/STOLEN PLATE AFFIDAVIT

When Does My License Plate Need to be Surrendered?

  • When insurance is canceled on a motor vehicle
  • When a vehicle is sold and the license plate is not being transferred to a replacement vehicle

How Long Do I Have to Surrender My License Plate?

30 days from the date the vehicle was sold or 30 days from the date insurance on the vehicle was canceled.

What Happens if I Do Not Surrender My License Plate?

Failure to surrender your license plate when you sell your vehicle and are not transferring the plate to a replacement vehicle will result in a driver license suspension. When you surrender your plate, a new vehicle registration will be printed indicating the license plate is canceled. Retain the “Surrendered Registration” receipt. This may be used to exempt the $225 initial registration fee when a replacement vehicle is registered.

What if I Do Not Have the License Plate Anymore?

If the license plate is not available for surrender, complete this affidavit and send to the address above along with a photo copy of your ID.

If your driver license is pending suspension due to cancellation of service, an office visit may be required. Contact us for more information.
